Bladder Cancer – Treatment Options
The first step in treating bladder cancer is usually a procedure called TURBT (Transurethral Resection of Bladder Tumor).
In this procedure, a special instrument is passed through the urethra into the bladder, and the tumor is removed endoscopically.
This not only treats the tumor but also helps determine its grade and stage in order to plan further management.
If the tumor is superficial and hasn’t invaded the bladder muscles, the bladder can usually be preserved.
In such cases, regular cystoscopy every 3 to 6 months is essential to detect and treat any recurrence early.
Sometimes after surgery, medications like BCG or chemotherapy are placed in the bladder to reduce the chance of recurrence.
On the other hand, if the tumor has invaded the muscle, radical cystectomy (removal of the whole bladder) is usually required.
In certain cases, systemic chemotherapy or immunotherapy may be given before or after surgery, depending on the stage of the disease and the overall condition of the patient.
سرطان مثانه – گزینه های درمانی
اولین قدم در درمان سرطان مثانه معمولاً روشی به نام TURBT (برداشتن تومور مثانه از طریق مجرای ادرار) است.
در این روش ابزار مخصوصی از مجرای ادرار به مثانه منتقل می شود و تومور به روش آندوسکوپی خارج می شود.
این نه تنها تومور را درمان می کند، بلکه به تعیین درجه و مرحله آن به منظور برنامه ریزی برای مدیریت بیشتر کمک می کند.
اگر تومور سطحی باشد و به عضلات مثانه حمله نکرده باشد، معمولاً می توان مثانه را حفظ کرد. در چنین مواردی، سیستوسکوپی منظم هر 3 تا 6 ماه یکبار برای تشخیص و درمان زودهنگام عود ضروری است.
گاهی اوقات پس از جراحی، داروهایی مانند BCG یا شیمی درمانی در مثانه قرار داده می شود تا احتمال عود بیماری کاهش یابد.
از طرف دیگر، اگر تومور به عضله حمله کرده باشد، معمولاً سیستکتومی رادیکال (برداشتن کل مثانه) مورد نیاز است.
در موارد خاص، بسته به مرحله بیماری و وضعیت کلی بیمار، ممکن است شیمی درمانی سیستمیک یا ایمونوتراپی قبل یا بعد از جراحی انجام شود.
